Medication reviews
This service ensures safe, effective medication use for aged care residents. Home Medicines Review (HMR) involves thorough medication reviews, while the Aged Care On-site Pharmacist (ACOP) places pharmacists on-site to provide direct support, education, and oversight. These services help reduce medication-related issues and enhance care continuity in aged care settings.
Diabetes Education
People living with diabetes may be eligible for Medicare rebates for diabetes education services provided under a GP referral as part of a Chronic Disease Management plan. Services include individual consultations, group diabetes education programs for people with type 2 diabetes, and follow-up consultations for Aboriginal and Torres Strait Islander people, with both face-to-face and telehealth options available.
Frequently asked questions for Doctors and Patients
The HMR Program, funded by the Australian Government Department of Health and Aged Care, supports the quality use of medicines by allowing patients to receive a comprehensive review of their medications at home. A Credentialed Pharmacist assesses the patient’s medication regimen, identifies any medication-related issues, and provides recommendations to optimise safety and effectiveness.
Patients eligible for an HMR must:
Hold a Medicare or DVA card
Live in a community (non-hospital) setting
Be at risk of or experiencing medication-related issues
Have a medical need for a review as determined by a referring doctor (General Practitioner [GP], specialist in Pain Medicine, Psychiatry, Palliative Medicine, or Specialist Physician)
A referral from the patient’s GP or other authorised medical practitioner.
A face-to-face interview with a credentialed pharmacist at the patient’s home will discuss medication usage, potential issues, and medication management.
The pharmacist’s assessment and report are sent to the referring doctor, outlining medication-related findings and recommendations.
A medication management plan is developed between the patient and doctor, with the option for follow-up interviews if needed.
Improve the safety and effectiveness of medication use
Address any medication issues that impact health outcomes
Enhance understanding of medications for both patients and healthcare professionals
Promote collaboration among the healthcare team for the patient’s benefit
Generally, a patient may have one HMR every 24 months. However, an additional HMR may be provided if a patient has significant changes in their health, such as:
Recent hospital discharge with medication changes
New or altered medical conditions
New medications requiring close monitoring
HMR services are provided at no cost to eligible patients with a Medicare or DVA card. The program is fully funded by the Australian Government.
Follow-up visits can be scheduled if clinically required to reassess the patient’s medications. The first follow-up is done within one to nine months after the initial interview, with a possible second follow-up if necessary.
A patient, caregiver, pharmacist, or community nurse may suggest an HMR to a GP. If the GP agrees it is necessary, they will provide a referral [Relevant Medicare Benefits Schedule - Item 900]. The Credentialed Pharmacist will then arrange the review at the patient’s home.

With over a decade of cumulative experience in pharmacy and dentistry, multidisciplinary approach to healthcare is my ethos. I bridge clinical expertise in both fields to deliver comprehensive, patient-centred care. My work spans community and hospital settings, with special interest in pain, oncology, haematology, and geriatric medicine. I manage complex medication and oral health needs and ensure optimal treatment plans for each individual.
In addition to my clinical practice, I have served in multiple academic, research, and editorial roles. These roles have significantly influenced my approach to evidence-based care and patient education, reinforcing my commitment to continuous learning and improvement.
